Job Summary: The Licensed Practical Nurse (LPN) in a long-term care facility is a vital member of the healthcare team, responsible for delivering high-quality nursing care to residents. The LPN works under the supervision of the RN and collaborates with other healthcare professionals to ensure the well-being of residents while adhering to relevant regulations and standards.

Key Responsibilities:
Patient Care:
· Perform nursing assessments and contribute to the development of care plans.
· Administer medications and treatments as prescribed.
· Monitor residents' health status and report changes to the RN Supervisor.
Supervision and Coordination:
· Oversee and guide Certified Nursing Assistants (CNAs) and other nursing staff.
· Collaborate with the interdisciplinary team to coordinate and provide comprehensive care.
Wound Care and Procedures:
· Conduct wound care and other nursing procedures.
· Monitor and document the progress of residents' conditions.
Documentation:
· Maintain accurate and up-to-date documentation of patient care and assessments.
· Ensure compliance with facility policies and regulatory requirements.
Medication Management:
· Administer medications according to physician orders and facility protocols.
· Monitor residents for potential side effects and drug interactions.
Communication:
· Effectively communicate with residents, families, and other healthcare team members.
· Provide education to residents and families about their conditions and care plans.
Infection Control:
· Follow and enforce infection control protocols.
· Educate staff on proper infection prevention measures.
Emergency Response:
· Respond to emergencies and provide assistance during crisis situations.
· Ensure the safety of residents and staff.
Quality Improvement:
· Participate in quality improvement initiatives.
· Contribute to data collection and analysis for continuous improvement in patient care.
Staff Training and Development:
· Provide orientation and ongoing education for nursing staff.
· Stay updated on industry developments and incorporate best practices into care delivery.
